Position Overview & Specifications

The Living Well Service enables timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is multidisciplinary, and our VCSE partner is Family Action BAND.

Bolton Living Well Service is compassionately led, trauma informed, and recovery focused. We are also be developing peer mentor opportunities for people with lived experience. CPD opportunities are available and personal interests/skills will be supported to develop and grow.

Living Well has co-produced key values and principles that all staff and peers share. As a result, our model is a humanistic and person-centred model of care. Our core values are based on creating open, honest, safe spaces and conversations where all people’s strengths are valued.

We are looking for an enthusiastic, motivated individual to work as a Trainee/Advanced Nurse Practitioner within the Living Well model. The post holder will also support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.

The post holder will be an integral part of the Living Well model supporting clinical decision making and supporting service pathways, developing nurse led clinics and providing brief interventions. The post holder will also support the development of pathways within Living Well as well as seamless transitions into & out of other available mental health services.

The post holder will hold a case load and work collaboratively within the Living Well team and with the wider network. The post holder may additionally lead on practice and service developments. The post holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

The post holder will have a good grasp of the community transformation programme, what Living Well is and the theory and background for this new approach. The post holder will have the ability to recognise people as the expert in their condition and have experience of working in a holistic manner. The post holder will work collaboratively with service users and include the person in all decisions about their care. You will have experience of co-producing personal recovery and action plans. You will have excellent knowledge of VCSE organisations and ideally have experience of working in partnership with VCSE organisations.

To ensure the continuity of the delivery of the LW Service for those people referred.

To assist in leading the service and promote a culture based on principles of recovery, choice and personalised care.

To work in an effective partnership with the MDT and other partner agencies including statutory services.

Ensure that service users ‘flow’ through pathways of support are in accordance with need. This includes being accountable for delivery of a proactive caseload management system.

To ensure the service works in line with agreed pathways, National and Trust policies, and guidelines, and is focussed on delivery of outcomes.

To assist with the management of the operational team, ensuring effective training, ongoing professional development and professional supervision is maintained for all staff.

To assist with managing financial resources efficiently and in line with agreed plans/budgets

To understand and contribute to the management of the wider environment and stakeholders – e.g. ICB.

To assist the Operational Programme Manager in the strategic development of the LW in Bolton and across organisation boundaries.
